Schedule FA from Shareworks (Solium)

Shareworks by Morgan Stanley (atwork.morganstanley.com/solium or shareworks.solium.com). Upload your holdings report below and it's read automatically — or, if your employer's portal offers no export, enter your vest lots by hand or let AI prefill them from your Statement of Account PDF.

Scope: US stock-plan accounts, USD values — RSU vest lots and ESPP purchases. Option exercises and non-US accounts aren't supported here yet.

Upload your Shareworks export (fastest)

Activity → Reports → Statement of Account over All Available History, downloaded as CSV or Excel, is the one file that carries both your lots and your sales. A Holdings by Lot, Holdings by Tranche or Summary of Stock Holdings export works too, as do the Releases, RSU Sales, ESPP and Dividend reports — add as many as you have and they'll be read together. If the report asks which currency to run in, choose US Dollars: a rupee-denominated report states no exchange rate, so those rows have to be skipped.

or, if your portal has no export

Prefill from your statement (optional)

Shareworks → Activity → Reports → run All Available History as a Statement of Account and download as PDF. Text-based PDFs only — scanned/photographed statements aren't supported yet.

Account details
Vest lots (required — at least one)
Vest date
Quantity
FMV per share (USD)
Description (optional)
ESPP purchases (optional — only if you have an Employee Stock Purchase Plan)

Enter the price you actually paid per share (after any employee discount) — the review page lets you choose FMV-on-purchase-date or this purchase price as the Table A3 basis.

Sales (optional — only if you sold shares)
Dividends (optional)

One row per credit. Only know the annual total? Enter a single row dated Dec 31 — it will convert at the Dec-31 SBI TTBR rate.

NRA tax withholding (optional — foreign tax withheld against a dividend)

One row per "Foreign Tax Withheld" / "NRA Withholding" line on your statement — usually dated the same day as a dividend credit. Used for Schedule FSI / Form 67 foreign tax credit; not required to file, but you'll otherwise under-claim the credit you're owed.
